Any idea what is wrong with my trees

I have something going on with my trees.
The branches are dying and the canopy is thinning. And like half the bark is falling off the trees. The trees are a Weeping Willow, maple, and Popular trees they all kind of have the same thing going on. No dirt has been added guess the planting to deep was a mistake I made. Also no heavy equipment has been around them. I was just wondering if it looked like a pest or fungus problem. If it’s from planting too deep I can remove some of the dirt around the bottom of the ones in good shape and replant with new ones were the bark is half gone unless someone has a better ideal.
I see past damage and possible poor planting - root flair too deep. Was soil added around the trees in the past? Any heavy equipment run nearby?
A mulch ring might help as the grass around the bases is competing for water and nutrients.
