Hazard Tree Evaluation form?

Does anyone use a standard form to evaluate the risk of failure of a hazard tree. I know there is a lot of experience involved in judge the chance of failure, but I am looking to put a quantitative value to the risk and give it to the customer based on actual measurements and probings. Something that can be evaluated anually forany changes.

I found this form, and it seems fairly comprehensive, but I would like any input you guys have. I have attached the form HERE
