How bad is this scoring on my cs-590?

That looks like a single score on the intake side... probably ingested a bit of crap at some point. I'd check the rings/piston through the intake, if that looks ok & it's still running good then probably not worth doing much else
 
Other than it's filthy, looks fine to me. Every piston after some running time, will exhibit some 'score' marks but those superficial ones in no way adversely impact the performance of the saw. Having said that, if the cooling fins are loaded with the gummy swarf present in the picture, I'd say you need to clean the saw up. A clean jug is a cool running jug.
 
Best to richen up high speed a bit to help prevent and future scoring. I always find echos are running ragged edge lean to pass emissions
