Safe Work Practices - work near water

Hey, folks.
One of our clients has asked for our safe work practices, including our practice for work near water. We have over 100 practices and procedures, but until now I had never considered working near water. It's a valid concern, and deserves a work practice document. Rather than re-invent the wheel, I would sure appreciate if anyone had one to share, or even the start of one I could modify.

The ultility I work for has a "work near water standard" in place. I beleive this is an OSHA requirement.

If we are working above or near water...even 2". Everyone that is exposed is supposed to wear a life jacket. The job site is to have a life saving ring attached to a rope which is tied off to a secure anchor point. A life skif or row boat is supposed to be on the job site. I'm sure there is other things I missed but thats the majority of things required.
As you know this sounds good on paper but in the "real world" it doesn't happen very often.
 
Life jacket for 2" water! wow! Wonder what that person was thinking when they wrote that.
This is a valid concern. I live/work around the Mississippi river all the time. Never thought about a boat on a job. I would like to see some as well if anybody has them.
