Starter rope breaking while saw is running?

Had a customer call me today with an issue he's having with a Stihl MS390. Seems the starter rope is somehow catching while the saw is running and it breaks the rope (about three times thus far)
I suggested he wind the spring a bit tighter and he said he already tried that.

I have seen reference to this happening on the forum before but a search didn't turn up anything. If someone has input on this, I'd appreciate it. (I've never encountered it)

One of the starter pawl springs may have broken or become displaced

Once the saw gets here, I'll look for that. Not sure if this one has the single or double pawl. (I've seen both)

Or a flywheel fin has broken off, thus no longer limiting the pawl.
