Sometimes you just gotta hope that common sense prevails. Not everything can be fool proofed. Thats why you usually see all the safety and "don't do" stuff is the very first section of many operating manuals.
We are talking here about heating elements being place in very close proximity to thermoplastic , which sometimes begins to deform in the 180 degrees F range, depending on type.
I suspect there was a stack-up of assembly positioning tolerences that allowed a section of the element to be a little to close to the plastic. It could also simply be that they have a marginal design situation.
